Wish I had a Nikon though... you can get off camera leads with AF-illumination LEDs in!
It's a bit of a pain focusing with the flash off the camera as you've basically got to somehow point the flash at the focus point you want to use, then look through the viewfinder and press the shutter down half-way. Hopefully you've aimed right and it will focus, otherwise it just un-focuses all the way so you can see nothing through the viewfinder - helpful!

The best way to do it is wireless flash!
Either set the flash on the camera to a low-power fill-in effect, or just set it not to fire and use the hand-held one for the primary subject illumination.

I'd love to try out the 2-flash setup, WITH that Sigma 8mm fisheye lens...
that would be awesome - they depth would be amazing!

hmm, nutta, you might wanna play with the next toy I'm looking at then, the wireless transmitter, has the af assist beam built in. cheaper for me to go down that route and get more flashes later.

:o)

I was looking at it... but decided against as for the same money I can have a 420ex... which (as I have a 580ex) would achieve the same result, AND at the same time be an extra flash!

It also means setup changes are quicker... just flick the 580 back to normal mode and put the other flash in your pocket, no un-mounting the transmitter and re-mounting the flash (a complex "3-handed" operation!)

hmm, true, but the difference in price between the 580 and the transmitter is quite a bit, had I got the 580 in the first place, it wouldn't have been an issue, but I'm building up my kit so I may end up getting a 580 in teh end as well anyway.

I used to get the same effect by mounting the flash on the camera - BUT NOT LOCKING IT!
Obviously no jumping about at this stage...
setup the shot and focus, don't press the shutter all the way down.
now, without changing the distance (or note the distance and maintain it) between you and the subject, remove the flash and fire it manually just after you press the shutter down again (remembering to switch the focus to MF after it has focused in the first part of the trick).

Helpful tips: use MANAUL mode! stop the lense down to increase depth of field, so precise focus (ie... precise distance between camera and subject) isn't as crucial.
Depending on how fast you can fire the flash after the camera, use an exposure time of between 0"3 and 0"8 - remember that a long exposure with the lense stopped down isn't going to be a problem anyway as it won't blur the pic.
You'll need to experiement with the flash power setting to get it right (kinda trial and improvement) - once you've worked it out, you shouldn't need to change it throughout the night unless you are shooting someone dressed darker/lighter than usual.
